Claim my refund
You recently received a letter from ANZ about your eligibility for a refund in connection with an account you had with ANZ.
To claim your refund, you will need to provide your full name, Date of Birth, Australian Bank Account details, telephone and email address into “My Details” section of this form.
By submitting your details you are confirming:
- You have authority to claim the refund.
- You agree that ANZ will collect and use your details to process your claim, and pay your refund to an eligible account according to your directions, including for the purposes of any administrative or operational processes required for the payment to be processed and for security purposes.
- You understand that an eligible account where the refund can be transferred is the one held by you, individually or jointly with the holder of an Australian Financial Services Licence, which is capable of accepting deposits in Australian currency.
- You agree that ANZ will disclose your details to ANZ’s service provider, Qualtrics to assist ANZ with the collection and storage of your details, and the processing of your refund. Qualtrics will store your details on secure cloud servers located in the USA.
